Description:
Inflammatory bowel disease (IBD) is a chronic, progressive, and lifelong disease characterized by severe inflammation and mucosal destruction in the intestine, encompassing Crohn's disease and ulcerative colitis. Crohn's disease causes chronic inflammation of the large intestine and ulcers in the lining of the large intestine and rectum. Ulcerative colitis (UC) causes chronic inflammation in all parts of the gastrointestinal tract from mouth to anus. It is estimated that 2.39 million Americans have IBD, including 1.25 million with UC and 1.01 million with Crohn's disease. Immediate goals are to relieve the symptoms to help improve a patient's quality of life, while long-term goals include maintenance of a corticosteroid-free clinical and endoscopic remission, prevention of complications, and intestinal mucosal healing. Diagnosing IBD can involve endoscopy and histopathological examination, but the assessment of biomarkers tends to be favored by patients due to the non-invasive nature, simplicity, speed, and the objective insight they provide. Therapeutic advances are transforming outcomes and treatment goals for many people living with IBD, but patients continue to experience a myriad of disparities in care that put them at an unequal risk for disease complications and poor outcomes. Addressing new and emerging clinical decision support tools can help providers bridge this gap by predicting individualized disease complication risk and appropriate responses to therapy.

Upon completion of this activity, participants will be able to:

• Explore personalized treatment strategies, based on current practice guidelines, to optimize the care and management of patients with inflammatory bowel disease (IBD)
• Examine the best diagnostic strategies using biomarkers and imaging for determining the severity of IBD
• Appraise the safety, efficacy, and mechanisms of action for novel biologic therapies, with a close look at the use of tumor necrosis factor-like cytokine 1A (TL1A), and their potential therapeutic impact across the IBD treatment paradigm
• Discuss shared decision-making strategies for biologic therapies to monitor and manage adverse events to address challenges for better adherence and access to treatments in IBD
• Integrate managed care strategies to ensure the evidence-based use of current and emerging biologics while informing formulary decisions, pathways, and cost effectiveness
